First Visit

Becoming a patient of Dr. Sullivan's is as easy as picking up the phone or sending us an email.

We welcome new patients into our practice through a variety of ways.

  • You may make an appointment for a regular cleaning and exam with one of our hygienists and Dr. Sullivan will meet you and provide a thorough exam at this time. This is best for patients that have been regular in their dental care.
  • You may make an appointment with Dr. Sullivan for a complimentary consultation visit. This is best if you are seeking a second opinion, feel as if you have some pending dental needs, or would just like to meet Dr. Sullivan and the staff.
  • You may also visit us for any dental emergency needs that you have. Our office is adept and getting patients comfortable, pain-free, and on the road to better dental health.

Please assist us by providing the following information at the time of your appointment:

  • Any  insurance information that you may have. Please bring your most current insurance card to expedite confirmation of coverages and co-pays. We file all claims electronically and current information allows us to be more certain of any questions that you may have.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • A list of medications you are presently taking.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to  your visit (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. Please contact the front office for electronic forwarding information.
