New Patient Forms

I was able to make my appointment online and fill out all the required forms before I got there. They offer early appointments and schedule enough time that you dont feel rushed. They asked more than once about my preferences Id listed when requesting the appointment and were very accommodating. Office was clean, good music and kind staff. Kayla and Dr Keely talked me through every step in detail and Sydnie did an excellent job describing my payment options. Thanks ladies.

I’ve had some extremely major work done by Sullivan Dental Partners however this time it was only a cleaning, I felt extremely safe and my hygienist was extremely confident. The vibe is comfortable and laid-back and as usual I feel good being in the dentist’s office!Highly recommend Sullivan Dental Partners! First let me say I am absolutely afraid to sit in a dentist chair. Since I have been going to Dr Rick, both her and and his staff has been amazing!!! I have had extensive dental work done by Sullivan dental. Their compassion for their patients goes way far and above. When I have needed to be seen, I have never had to wait.
